Development of an artificial intelligence diagnostic model based on dynamic uncertain causality graph for the differentialdiagnosis of dyspnea

摘要: Dyspnea is one of the most common manifestations of patients with pulmonary disease, myocardial dysfunction, and neuromuscular disorder, among other conditions. Identifying the causes of dyspnea in clinical practice, especially for the general practitioner, remains a challenge. This pilot study aimed to develop a computer-aided tool for improving the efficiency of differential diagnosis. The disease set with dyspnea as the chief complaint was established on the basis of clinical experience and epidemiological data. Differential diagnosis approaches were established and optimized by clinical experts. The artificial intelligence (AI) diagnosis model was constructed according to the dynamic uncertain causality graph knowledge-based editor. Twenty-eight diseases and syndromes were included in the disease set. The model contained 132 variables of symptoms, signs, and serological and imaging parameters. Medical records from the electronic hospital records of Suining Central Hospital were randomly selected. A total of 202 discharged patients with dyspnea as the chief complaint were included for verification, in which the diagnoses of 195 cases were coincident with the record certified as correct. The overall diagnostic accuracy rate of the model was 96.5%. In conclusion, the diagnostic accuracy of the AI model is promising and may compensate for the limitation of medical experience.

摘要: The aim of this study was to investigate the value of technetium etifenin injection ( Tc-EHIDA) hepatobiliary scintigraphy in combination with determination of bilirubin from duodenal drainage in differential diagnosis between infantile hepatitis syndrome and biliary atresia. Tc-EHIDA hepatobiliary scintigraphy in combination with duodenal fluid examination was used for evaluation in 84 infants with persistent infantile jaundice. For diagnosing biliary atresia, the sensitivity and specificity of scintigraphy were 100% and 74.5%, respectively; the sensitivity and specificity of scintigraphy in combination with duodenal fluid examination were 100% and 100%, respectively. In conclusion, hepatobiliary scintigraphy, which is a noninvasive, safe, valuable examination method, in combination with examination of duodenal fluid, is of value for the differential diagnosis between infantile hepatitis syndrome and biliary atresia.

The value of ultrasound elastography in differential diagnosis of superficial lymph nodes

摘要: The study evaluated the value of ultrasound elastography in differentiating the benign and malignant superficial lymph nodes. A total of 112 subjects, including 82 patients with enlarged lymph nodes and 30 healthy volunteers, were recruited. All the subjects were examined by B-mode ultrasonography, power Doppler ultrasonography and elastography. Most of the patients were histopathologically confirmed by needle aspiration cytology and some patients were diagnosed by clinical data combined with follow-up findings. The sensitivity, specificity and accuracy of B-mode ultrasonography were 59.8%, 76.5% and 67.1%, those of the blood flow classification by power Doppler ultrasonography 77.0%, 82.3% and 79.4% and those of elastographic classification 74.7%, 97.1% and 84.5%, respectively. The elasticity of the lymph nodes was quantitatively measured and defined as stiffness value. When the stiffness value of 2.395 was taken as the cutoff point, the sensitivity and specificity of elastography were 78.41% and 98.51%, and the Youden index reached the highest, with the value being 0.7692. The stiffness values of two indeterminate benign lymph nodes, the elastrographic findings of which were rated as patterns 2 and 3, were below the cutoff point. The elastographic findings of 10 malignant lymph nodes were also classified as pattern 2 or 3. Only one of them had the stiffness values below the cutoff point. It was concluded that ultrasound elastography is a novel, noninvasive and convenient tool for the differentiation of the nature of the superficial lymph nodes in clinical practice.

Differential diagnosis of cervical nerve compression syndrome of the external intervertebral foramen

摘要: The aim of the present research is to study the mechanism of cervical nerve compression syndrome of the external intervertebral foramen and its differential diagnosis with cervical spondylosis. Diagnostic treatment with muscle relaxant, vasodilator, neurotrophic medicine and celecoxib (COX)-2 inhibitor were performed in 20 patients with cervical nerve compression syndrome of the external intervertebral foramen and 20 patients with cervical spondylosis confirmed by operation. Diagnostic local block therapy was performed additionally in cases showing little effect after diagnostic treatment. All the patients were followed up postoperatively for more than one year. Fifteen cases with cervical nerve compression syndrome of the external intervertebral foramen were healed by the diagnostic treatment. The other five cases had a short-term remission and there was no recurrence after diagnostic local block therapy. Diagnostic treatment led to short-term alleviation of the symptom in 20 cases with cervical spondylosis confirmed by operation, the results of which was far from satisfactory and operation was undertaken finally in all the 20 cases. The etiology of cervical nerve compression syndrome of the external intervertebral foramen lies in the compression of the cervical plexus, brachial plexus and cervical dorsal rami by the tendinous decussating fibers of the scalenus anticus, medius, minimus and the posterior muscles of the neck. Diagnostic treatment was propitious to differentiate cervical nerve compression syndrome of the external intervertebral foramen from cervical spondylosis.

Trauma-induced “Macklin effect” with pneumothorax and large pneumomediastinum, disguised by allergy

摘要:

A 56-year-old man presented spontaneously to the Emergency Department complaining of facial and neck oedema after assumption of nonsteroidal anti-inflammatory drugs (NSAIDS). The triage nurse assigned the patient to Accident & Emergency (A&E) doctor as probable allergic reaction to NSAIDS. Chest X-ray (CXR), ordered after 24 hours, revealed a huge subcutaneous chest and neck emphysema without clearly visible pneumothorax. Subsequent chest CT scan showed a small left pneumothorax and a large amount of air in the mediastinum. The patient was conservatively treated since he was eupnoeic and hemodynamically stable. The pathophysiology of pneumomediastinum was first described by Macklin in 1939. The Macklin effect involves alveolar ruptures with air dissection along bronchovascular sheaths to the mediastinum. In this case the patient did not report in his history a recent blunt thoracic trauma and the initial suspicion of an allergic reaction has prevented physicians to immediately achieve the correct diagnosis.

摘要: Brucellosis is a worldwide zoonosis. Vaccination is the most efficient means to prevent and control brucellosis. The current licensed attenuated vaccines for animal use were developed by sequential passage in non-natural hosts that decreased virulence in its original hosts. The attenuation mechanism of these strains remains largely unknown. In the present study, we sequenced the genome of vaccine strain M5-10. Sequence analysis showed that a large number of genetic changes occurred in the vaccine strains. A total of 2854 genetic polymorphic sites, including 2548 SNP, 241 INDEL and 65 MNV were identified. Of the 2074 SNPs in coding regions, 1310 (63.2%) were non-synonymous SNPs. Gene number, percent and N/S ratios were disproportionally distributed among the cog categories. Genetic polymorphic sites were identified in genes of the operon, flagella synthesis, and virulence regulating systems. These data indicate that changes in some cog categories and virulence genes might result in the attenuation. These attenuation mechanisms also have implications for screening and development of new vaccine strains. The genetic changes in the genome represent candidate sites for differential diagnosis between these vaccine strains and other virulence ones. Transcription analysis of virulence genes showed that expression of , were reduced in M5-10 strain when compared with that in 16M. A duplex PCR targeting and was successfully used to differentiate between M5-10 and the virulent 16M strain. The genome re-sequencing technique represents a strong strategy not only for evaluation of vaccines, but also for development of new vaccines.

Tuned reactive power dispatch through modified differential evolution technique

摘要: This paper explores the capability of modified differential evolution (MDE) technique for solving the reactive power dispatch (RPD) problem. The proposed method is based on the basic differential evolution (DE) technique with a few modifications made into it. DE is one of the strongest optimization techniques though it suffers from the problem of slow convergence while global minima appear. The proposed modifications are tried to resolve the problem. The RPD problem mainly defines loss minimization with stable voltage profile. To solve the RPD problem, the generator bus voltage, transformer tap setting and shunt capacitor placements are controlled by the MDE approach. In this paper, IEEE 14-bus and IEEE 30-bus systems are chosen for MDE implementation. The applied modification show much improved result in comparison to normal DE technique. Comparative study with other soft-computing technique including DE validates the effectiveness of the proposed method.

Review of MEMS differential scanning calorimetry for biomolecular study

摘要:

Differential scanning calorimetry (DSC) is one of the few techniques that allow direct determination of enthalpy values for binding reactions and conformational transitions in biomolecules. It provides the thermodynamics information of the biomolecules which consists of Gibbs free energy, enthalpy and entropy in a straightforward manner that enables deep understanding of the structure function relationship in biomolecules such as the folding/unfolding of protein and DNA, and ligand bindings. This review provides an up to date overview of the applications of DSC in biomolecular study such as the bovine serum albumin denaturation study, the relationship between the melting point of lysozyme and the scanning rate. We also introduce the recent advances of the development of micro-electro-mechanic-system (MEMS) based DSCs.

Laboratory diagnosis for malaria in the elimination phase in China: efforts and challenges

摘要: Malaria remains a global health challenge, although an increasing number of countries will enter pre-elimination and elimination stages. The prompt and precise diagnosis of symptomatic and asymptomatic carriers of Plasmodium parasites is the key aspect of malaria elimination. Since the launch of the China Malaria Elimination Action Plan in 2010, China has formulated clear goals for malaria diagnosis and has established a network of malaria diagnostic laboratories within medical and health institutions at all levels. Various external quality assessments were implemented, and a national malaria diagnosis reference laboratory network was established to strengthen the quality assurance in malaria diagnosis. Notably, no indigenous malaria cases have been reported since 2017, but the risk of re-establishment of malaria transmission cannot be ignored. This review summarizes the lessons about malaria diagnosis in the elimination phase, primarily including the establishments of laboratory networks and quality control in China, to better improve malaria diagnosis and maintain a malaria-free status. A reference is also provided for countries experiencing malaria elimination.

Solving multi-objective optimal power flow problem considering wind-STATCOM using differential evolution

摘要: In this paper, a simple strategy based differential evolution was proposed for solving the problem of multi-objective environmental optimal power flow considering a hybrid model (Wind-Shunt-FACTS). The DE algorithm optimized simultaneously a combined vector control based active power of wind sources and reactive power of multi STATCOM exchanged with the electrical power system to minimize fuel cost and emissions. The proposed strategy was examined and applied to the standard IEEE 30-bus with smooth cost function to solve the problem of security environmental economic dispatch considering multi distributed hybrid model based wind and STATCOM controllers. In addition, the proposed approach was validated on a large practical electrical power system 40 generating units considering valve point effect. Simulation results demonstrate that choosing the installation of multi type of FACTS devices in coordination with many distributed wind sources is a vital research area.

Biosensor-based assay of exosome biomarker for early diagnosis of cancer

摘要: Cancer imposes a severe threat to people’s health and lives, thus pressing a huge medical and economic burden on individuals and communities. Therefore, early diagnosis of cancer is indispensable in the timely prevention and effective treatment for patients. Exosome has recently become an attractive cancer biomarker in noninvasive early diagnosis because of the unique physiology and pathology functions, which reflects remarkable information regarding the cancer microenvironment, and plays an important role in the occurrence and evolution of cancer. Meanwhile, biosensors have gained great attention for the detection of exosomes due to their superior properties, such as convenient operation, real-time readout, high sensitivity, and remarkable specificity, suggesting promising biomedical applications in the early diagnosis of cancer. In this review, the latest advances of biosensors regarding the assay of exosomes were summarized, and the superiorities of exosomes as markers for the early diagnosis of cancer were evaluated. Moreover, the recent challenges and further opportunities of developing effective biosensors for the early diagnosis of cancer were discussed.

Cluster voltage control method for “Whole County” distributed photovoltaics based on improved differential

摘要: China is vigorously promoting the “whole county promotion” of distributed photovoltaics (DPVs). However, the high penetration rate of DPVs has brought problems such as voltage violation and power quality degradation to the distribution network, seriously affecting the safety and reliability of the power system. The traditional centralized control method of the distribution network has the problem of low efficiency, which is not practical enough in engineering practice. To address the problems, this paper proposes a cluster voltage control method for distributed photovoltaic grid-connected distribution network. First, it partitions the distribution network into clusters, and different clusters exchange terminal voltage information through a “virtual slack bus.” Then, in each cluster, based on the control strategy of “reactive power compensation first, active power curtailment later,” it employs an improved differential evolution (IDE) algorithm based on Cauchy disturbance to control the voltage. Simulation results in two different distribution systems show that the proposed method not only greatly improves the operational efficiency of the algorithm but also effectively controls the voltage of the distribution network, and maximizes the consumption capacity of DPVs based on qualified voltage.

Tomographic diagnosis of defects in hydraulic concrete structure

摘要: The ultrasonic tomographic technology is applied to diagnose the defects in hydraulic concrete structure. In order to improve the precision of diagnoses, the wavelet transformation is used in the processing of ultrasonic signals. The influences of water, scale and orientation of defect, processing methods and theoretical model on image resolution are investigated. The experimental results indicate that the result of the tomographic diagnosis of a single defect is sensitive and the boundary can be clearly determined. However, the image resolution of multiple defects is not satisfactory. The water content and scale of a defect may significantly affect the imaging resolution. Defects with the orientation perpendicular to the direction of the diagnosis may have higher precision in diagnosing. The wavelet transformation technology can elevate the imaging resolution. The applied calculation model plays a very important role in improving the accuracy of detection.

The kinematic assumptions upon which the Euler-Bernoulli beam theory is founded allow it to be extended to more advanced analysis. Simple superposition allows for three-dimensional transverse loading. Using alternative constitutive equations can allow for viscoelastic or plastic beam deformation. Euler-Bernoulli beam theory can also be extended to the analysis of curved beams, beam buckling, composite beams and geometrically nonlinear beam deflection. In this study, solving the nonlinear differential equation governing the calculation of the large rotation deviation of the beam (or column) has been discussed. Previously to calculate the rotational deviation of the beam, the assumption is made that the angular deviation of the beam is small. By considering the small slope in the linearization of the governing differential equation, the solving is easy. The result of this simplification in some cases will lead to an excessive error. In this paper nonlinear differential equations governing on this system are solved analytically by Akbari-Ganji’s method (AGM). Moreover, in AGM by solving a set of algebraic equations, complicated nonlinear equations can easily be solved and without any mathematical operations such as integration solving. The solution of the problem can be obtained very simply and easily. Furthermore, to enhance the accuracy of the results, the Taylor expansion is not needed in most cases via AGM manner. Also, comparisons are made between AGM and numerical method (Runge-Kutta 4th). The results reveal that this method is very effective and simple, and can be applied for other nonlinear problems.

convolutional tree-inspired network: a decision-tree-structured neural network for hierarchical fault diagnosis

摘要: The fault diagnosis of bearings is crucial in ensuring the reliability of rotating machinery. Deep neural networks have provided unprecedented opportunities to condition monitoring from a new perspective due to the powerful ability in learning fault-related knowledge. However, the inexplicability and low generalization ability of fault diagnosis models still bar them from the application. To address this issue, this paper explores a decision-tree-structured neural network, that is, the deep convolutional tree-inspired network (DCTN), for the hierarchical fault diagnosis of bearings. The proposed model effectively integrates the advantages of convolutional neural network (CNN) and decision tree methods by rebuilding the output decision layer of CNN according to the hierarchical structural characteristics of the decision tree, which is by no means a simple combination of the two models. The proposed DCTN model has unique advantages in 1) the hierarchical structure that can support more accuracy and comprehensive fault diagnosis, 2) the better interpretability of the model output with hierarchical decision making, and 3) more powerful generalization capabilities for the samples across fault severities. The multiclass fault diagnosis case and cross-severity fault diagnosis case are executed on a multicondition aeronautical bearing test rig. Experimental results can fully demonstrate the feasibility and superiority of the proposed method.
